Root cause analysis (RCA) is a structured method of problem solving used for identifying the root causes of faults or problems.

Root cause analysis is a collective term that describes a wide range of approaches, tools, and techniques used to uncover causes of problems (breakdowns in processes and systems) and develop corrective actions to prevent future events

RCA is a structured facilitated team process to look deeper into problems identify root causes of an event that resulted in an undesired outcome and find out why they’re happening and develop corrective actions.

The purpose of an RCA is to find out what happened, why it happened, and determine what changes need to be made to prevent future events.

Key Points

Root Cause Analysis is a useful process for understanding and solving a problem.thus preventing recurrence.

RCA assumes that systems and events are interrelated. An action in one area triggers an action in another and.. so on. By tracing back these actions, you can discover where the problem started and how it grew into the symptom you’re now facing.

The process involves data collection, cause charting, root cause identification and recommendation generation and implementation.
