For THREE simple reasons:
- Problem is not understood
- Corrective Actions identified but they are never implemented
- Stopping the analysis too early
Problem definition should be quick and simple and should not take more than 10 minutes to define. If it does, it indicates other issues the team is struggling with. Hidden agenda, too complex, multiple event ..
What good is it if you identify Corrective Actions and you do not implement them? You have done more harm than good to your organization if you do this. Corrective Actions must have implementation owners, process owners and due date
Stopping the analysis because you think you got to the Root Cause or when you think you have a solution in hand is the worst thing you can do. You should STOP the analysis for the right reasons.
All of these can be addressed with the right corrective action program and right ownership.
